A Bachelor’s degree in a human services field, social work, rehabilitation or a bachelor’s of science in Nursing or possess an Associate’s degree with two years experience in providing human services, or five years of human service experience or possess a relevant professional credential, such as Certified Psychiatric Rehabilitation Practitioner, Certified Rehabilitation Counselor, or Certified Alcohol and Drug Counselor, Computer literacy required. Valid driver’s license and the ability to drive a van.
Provide case management services for clients as assigned, including individual goal setting, crisis intervention and coordination of medical, psychiatric, financial, residential, social, and legal needs. Assist clients in obtaining entitlements, especially Medicaid/Medicare, food stamps and medical records. Liaison with family, boarding home staff and local agencies which serve the elderly. Consult regularly with Prospect House program psychiatrist. Arrange transportation services and also provide transportation as needed.
In collaboration with staff, supervisors and clients, to provide partial care services to individuals with severe and persistent mental illness to enhance their community functioning. Plan and facilitate therapy, coping activity and psycho-educational groups to improve social skills which promote relapse prevention and independent living. Collaborate with clients and staff to accomplish psycho social goals and tasks of the unit. Provide milieu management. Participate in unit trips including van driving. Evening, weekend and holiday social program coverage as required by program needs. Maintenance of electronic clinical records including case notes, treatment planning, outcome data and other reports as required by funding sources. Participate in computer training as required. Participate in client advocacy and outreach/marketing efforts as required.
