Intake Coordinator – Prospect House

Qualifications
Master’s degree in Social Work, Psychology, Counseling or related field. Professional license preferred.  Experience working with a Psychiatric and MICA population.  Experience conducting intakes with strong clinical assessment skills.  Community outreach experience desired.  Valid Driver’s License.  Computer literacy required.

Responsibilities
Coordinate intake services with Medical and Program Director
Manage daily referral calls and maintain intake schedule
Schedule and conduct interviews/intake and assessment for services
Orient and engage new consumers in a culturally and linguistically appropriate manner
Meet with referral sources to coordinate and market partial services
Assist in community outreach to develop new referral sources
Provide program follow-up services and program integration
Document all information in electronic clinical record in a timely manner.
Track intake statistics to report monthly to Program Director
Must meet all expected productivity levels, in regard to direct service provision to consumers.
Perform other duties as assigned by the Program Director.
