Member Care Coordinator – Prospect House

Master’s degree in a Mental Health discipline. Supervisory experience preferred. Valid driver’s license and ability to drive van required. Knowledge of psychiatric disability, and psycho-social rehabilitation desirable. Computer literacy required.

Responsible for daily implementation of Partial Care Program in collaboration with staff, supervisors and members; to provide partial care services to individuals with severe and persistent mental illness; to enhance community functioning.

Program Development
Collaborate with Program Director to develop program growth and expansion of services, select supplies and materials, research new program methods and activities. Maintain contact with other programs for information sharing, referrals and public relations. Contribute to weekly scheduling of staff.

Supervision of Unit Staff
Provide supervision and support to the intake department assisting consumers with application for eligible entitlements including: Medicaid, Medicare and Social Security. Provide supervision and support to the billing departments, ensuring all prior authorizations are submitted in a timely fashion. Provide field supervision for graduate and undergraduate students and volunteers: serve as liaison with university clinical faculty. Supervise staff responsibilities including group activity facilitation, milieu management, case management and record keeping. Manage and provide support as needed to the Prospect Primary Care unit; including scheduling, follow up and testing.

Record Keeping
Record keeping as required by funding sources, through the electronic clinical record software and/or accreditation agencies, including case notes, group therapy notes, treatment notes, treatment planning, termination summaries, correspondence with insurance providers, outcome data.

Milieu Activities
Plan and facilitate therapy and coping activity and psycho-educational groups to enhance social skills and promote relapse prevention and independent living. Collaborative with members and staff to accomplish psycho-social goals and tasks of unit. Provide milieu management. Participate in unit trips including van driving, evening, weekend and holiday social program coverage as required by program needs. Investigation of incidents, preparation of incident reports; review incident report with Program Director, taking follow-up measures are required. Crisis intervention in coordination with Program Director.

Participate in computer training as required. Participate in outreach/marketing efforts as required. Public relations, referring agencies hospitals in community, as required. Participate in appropriate professional, family and civic organizations.

Must meet all expected productivity levels, in regard to direct service provision to consumers.

Perform other duties as assigned by the Director.
