Mental Health Forensic Case Manager

Minimum Qualifications
Bachelor’s Degree in a related field + 1 year of experience with SMI. Experience working with the Forensic population preferred.

Responsibilities
Provides community based outreach services to individuals involved in the criminal justice system. Completes assessments/reassessments, linkage, service coordination and post-release planning in the County Correctional Facility, individual’s home or other locations convenient for the individual. Develops treatment plans with consumers and coordinates services with multiple providers. Participates in team and community provider meetings.

Electronic Clinical Record keeping will be required as indicated by funding sources and/or accreditation agencies. Case managers must meet all expected productivity levels for direct service provision to consumers.
