Part Time Coordinated Entry Case Manager – Projects for Assistance in Transition from Homelessness

Qualifications
The Individual for this position must possess a Bachelor’s Degree from an accredited college or university in Social Work with at least 1-2 years of experience or a Bachelor’s Degree from an accredited college or university in a related field with at least 1-2 years of experience in a shelter, transitional or supportive housing. Must have a valid Driver’s License in good standing and have access to a vehicle.       

Responsibilities
The Coordinated Entry Case Manager will focus on identifying homeless individuals in need of shelter and will assist with linking them to the coordinated entry process. The Coordinated Entry Case Manager will act as the liaison between the client, and the case management entity.  Facilitate linkage to temporary shelter placement via 211 coordinated entry process.  Provide follow up services as needed by the Coordinated Entry Committee to individuals on waiting lists.  Coordinate with Housing Development Specialist for viable housing options and available subsidies. When appropriate, facilitate linkages to additional supportive services as identified in the assessment tool and interview. Crisis intervention.  Coordinate transportation needs and provide transportation as necessary. Collaborate with other participating agencies to ensure continuity of service delivery and assignment of a case manager at appropriate agency. Document all service delivery, maintain accurate records and maintain consumer confidentiality. Supports agency policies and implements procedures. Attends and cooperates with individual supervision. Seeks help from supervisor as needed. Participates in all scheduled in-service trainings, team member meetings.            

            

Special Skills
Must possess good oral and written communication skills. Knowledge of local community services. Individual must be detail oriented, efficiently manage and prioritize numerous tasks and complete work within deadlines. Knowledge of database applications. Able to work within a busy environment and remain focused and prioritize service delivery to consumers in need. Spanish speaking preferred. maintain accurate records, case notes and track progress in timely manner. Interacts with diverse populations in a supportive and respectful manner.
