Per Diem Case Worker – The Homeward Bound Project

Per Diem Back-Up Coverage for all Shifts – Monday through Sunday, Shifts – 8am-4pm, 4pm-12am, 12am-8am – Reply must include what days and shifts you are available.


Qualifications
A minimum of two (2) years of college with a hundred and twenty (120) college credits, with an emphasis in Social Work, Psychology, or related human services, or three (3) years’ experience working in outreach and referral services to homeless individuals seeking shelter. Valid Driver’s license.

Responsibilities
Daily activity reports, daily sign-in sheets and daily encounter reports and provides all reports to the Program Monitor; Provide outreach services implementing strengths-based, person-centered services; Provide outreach, engagement, case management and skill-building services; Provide case management and ongoing linkage to community supports such as mental health treatment, medical treatment, alternate shelter, and other needed services; Promote the integration of physical and mental health; Assist individuals with utilizing all available community supports that best meet their needs; Observe homeless individuals for indication of major problems (e.g. alcohol or substance abuse, psychiatric disorder, physical disability, etc.) to determine services that best meet their needs; Obtain information from homeless individuals as to their past housing history, including previous permanent residences, to determine if consumer is chronically homeless; Assist in the process of placement, including the providing of transportation of homeless; Maintain and upgrade clerical and professional skills; Complete all required documentation in clinical files; Attend supervision meetings and agency staff meetings; Attend meetings with program partners and community providers to reach program objectives; Must meet all expected productivity levels, in regards to the direct service; Perform other duties as assigned by the Program Director.

Skills Needed
Ability to assess general clinical status as well as interpret clinical charts when available; Ability to identify clinical crisis that may require emergency intervention;   Ability to complete a bio-psychosocial interview;Ability to engage with individuals;  Ability to network with a variety of both formal and informal community resources and supports;  Ability to educate individuals regarding mental illness and other functional impairments;  Ability to discuss substance abuse/dependence issues as well as educate individuals on these issues; Follow “safe driving” habits as identified by Motor Vehicle report; Have awareness of clinical resources available and means required to obtain them; Ability to assist individuals in creating a crisis plan with that utilizes all community 24 hour services;   Ability to educate individuals on how to follow a crisis plan; Ability to clearly communicate verbally and in writing.
