Part Time Registered Nurse – Projects for Assistance in Transition from Homelessness – (PATH)

Qualifications
Must have a current Registered Nurse certification in the State of New Jersey. Personal professional liability insurance and proof thereof. Valid Driver’s License.

Responsibilities
Assist with the integration of medical care for Case Management and Care Coordination addressing co-occurring physical health and behavioral health problems. Complete nursing assessments and reassessments; participate in the development of a wellness plan; provide education and consultation on medical and health maintenance issues; Address physical health problems via service planning and health and wellness skill-building. Provide support to Outreach Case Managers regarding outreach and engagement. Complete all required documentation in clinical files and in NJHMIS within a timely basis as per policies and applicable regulations. Attend supervision meetings and agency staff meetings; attend meetings with program partners and community providers to reach program objectives. Perform other duties as assigned by the PATH Program Director.

Special Skills
Ability to assess general clinical status as well as interpret clinical charts when available; to identify clinical crisis that may require emergency intervention; ability to complete general medical status as well as interpret medical charts when available. Ability to engage with individuals and educate individuals regarding medical conditions and how to follow a crisis plan.  Create a crisis plan with each individual that utilizes all community 24 hour services. Ability to clearly communicate verbally and in writing.
