Outreach Case Worker

Overnight Shift
12:00am – 8:00am, Monday through Friday

QualificationsIndividual must possess a minimum of two (2) years of college with a hundred and twenty (120) college credits, with an emphasis in Social Work, Psychology, or related human services, or three (3) years’ experience working in outreach and referral services to homeless individuals seeking shelter.

Must be able to identify clinical crisis that may require emergency intervention and engage with and educate individuals regarding mental illness and substance abuse/dependence issues.

Must have a current and valid Driver’s license.

ResponsibilitiesProvides outreach, engagement, case management and skill-building services.  Provides case management and ongoing linkage to community supports such as mental health treatment, medical treatment, alternate shelter, and other needed services. Observes homeless individuals for the indication of alcohol or substance abuse, psychiatric disorders or physical disability and determines which services will best meet their needs. Assists in the process of placement, including providing transportation to homeless individuals. Completes all required documentation in clinical files.

Outreach Case Workers must meet all expected productivity levels for direct service provision to consumers
