Registered Nurse – Community Support Services

Qualifications
Individual must possess license as a registered nurse (RN) applicable by the New Jersey state professional board and have graduated from an accredited nursing training program.

 Ability to work a flexible schedule, including evening, weekend and holiday hours.  Valid NJ driver’s license in good standing. Personal professional liability insurance and proof thereof.

Responsibilities
Offer services appropriate to meet the consumer’s needs. Perform therapeutic rehabilitative skill development. Contribute to the development, implementation, monitoring, and updating of the individualized rehabilitation plan. Coordinate and manage services.  Perform crisis intervention. Perform a comprehensive rehabilitative needs assessment. Completion of the nursing assessment, annual re-assessment and any clinically necessary re-assessments; Provision of education and consultation on medication and health maintenance issues and topics; Face-to-face health care monitoring at least every 90 days with each consumer resident; Contribution to staff education for the CARF health home accreditation. Record keeping as required by funding source through the electronic clinical record software, and/or accreditation agencies. Must meet all expected productivity levels, in regard to direct service provision to consumers. Perform other duties as assigned by the Director.
